BCBS, Carefirst, Anthem, UHC, VA-CCN
I work with The Lactation Network to provide coverage for many PPO Plans with BCBS, Carefirst, Anthem, Unitedhealthcare as well as VA Community Care Network (not BCBS or UHC HMO, BCBS EPO, Medicaid Community plans, or BCBS Federal). Check insurance coverage by clicking the button above. Both you and I will receive confirmation of coverage or denial. If you are covered, you are able to receive 6-8+ visits at no cost to you! Cigna
I work with Wildflower to provide coverage for Cigna plans including TPA (Third Party Administrator) plans. Check insurance coverage by clicking the button above. Once coverage is determined we will be connected in their portal and up to 6 visits will be submitted on your behalf at no cost to you! Please allow for email confirmation prior to scheduling youe appointment.
Aetna
I am In-Network with Aetna and bill lactation as a preventative service. Most plans cover this without cost sharing for the patient per the ACA Mandate but there may be a limit or fine print to be aware of. If you have Aetna please go to the button above and complete the form. I will be in touch within 24-48 hours with additional information. If not already confirmed, you will be responsible for verifying in-network status and your benefits.
Out-of-Network Insurances & Self Pay
I bill for Out-of-Network or Self Pay at $200 for an Initial Home Visit, $150 for a Followup Home Visit and $100 for a Virtual Visit. Individual Pre-Natal Education is $200 for In Home and $150 for Virtual (please note this differs than the group class).
I am able to provide a Superbill for reimbursement if I am out-of-network with your insurance. It cannot be guaranteed that you will be reimbursed.
Pre-Natal Individual Education
If a group class isn’t your thing or you have specific concerns, the individual pre-natal education visits are perfect for you. Insurance can be used for Individual Prenatal Education but not for group classes. Please be aware that any Pre-Natal Classes (whether with me or with a breast pump provider) count towards your limit if your plan is restricted in the number of visits.
Pre-Natal Group Education Classes
$40/Class. Support partners are invited! Not covered by insurances. This is a general class, more specific questions or plan creation is best suited for Individual Pre-Natal Education.
